Step - 1:-

Collect Page Content

Step - 2:-

Create Google Form Using Https://docs.Google.Com/forms/

(After completing your form move to next step)

Step - 3:-

Get Code of Form (through which the form will get embed into the page)

Right Top - Click on Send

Step - 4:-

<> (Code View) - Embed HTML - Copy Code

Step - 5:-

And Paste that Using Static Page - Give Page Name - URL - under

- Page Content - use Smart Tools - Html (paste your Embed HTML here)
